29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel.  On 5/20/2017, at the job site of 1020 Aviation Blvd, Gainesville, Georgia, employees were exposed to Ammonia exposure hazards as the employer did not develop and implement required safe work practices. Instances included:  a - The employer did not implement the documented Line Break policy and an employee was exposed to Ammonia when no personal protective equipment was donned for that event.   b - The employer did not develop specific written Lockout/Tagout procedures for employees replacing a portion of a 6 in. HSD line on an active Ammonia refrigeration system.   c - The employer did not provide locks to Authorized employees performing service/maintenance work on an active Ammonia refrigeration system.  d - The employer did not provide training in a Lockout/Tagout program and specific written procedures to employees performing service/maintenance work on active Ammonia refrigeration systems.  e - The employer permitted service tech employees to use hand held Acetylene torches to defrost ice from Ammonia lines that were to receive service and/or maintenance.

29 CFR 1910.119(h)(3)(i): 29 CFR 1910.119(h)(3)(I): The contract employer did not ensure that each contract employee is trained in the work practices necessary to safely perform his/her job.  On 5/20/2017, the employer did not train each employee in the work practices necessary as required by the standard and to ensure each employee could perform the job safely. i.e. Management officials did not train Alta employees in the MarJack contractor training program that included information on the following topics: Facility Specific Hazards, Hot Work Permits, Lockout/Tagout, Ammonia Refrigeration SOP Policy, Ammonia System Entry Guidelines, and Emergency Evacuation and Action Plan.

29 CFR 1910.140(c)(13)(ii): Fall protection system anchorages were not designed, installed, and used, under the supervision of qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two.  On 5/20/2017, at the job site located at 1020 Aviation Blvd., Gainesville, Georgia, the employer did not ensure employees using fall protection systems used anchorage points that were installed by a qualified person. Employees were installing fall protection systems by wrapping the lanyard around active Ammonia refrigeration system pipes and then back into the lanyard itself.
