29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):    a)     This violation was discovered on May 24, 2017, at the Kaufman, Texas facility; where the employer did not establish a program consisting of the energy control procedures and employee training for employee(s) servicing, maintenance and clearing jams on Sheffield cold knife cutting machine (model number C-530, serial number 5397).    b)    This violation was discovered on May 24, 2017, at the Kaufman, Texas facility; where the employer did not establish a program consisting of the energy control procedures and employee training for employee(s) servicing, maintenance and clearing jams on Sheffield heat knife cutting machine (model number HC-530, serial number 2147).

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):       a)     This violation occurred on or about May 15, 2017, at a job site located at 1072 East US Hwy. 175, Kaufman, Texas in the sewing department; where at least one (1) employee was exposed to an amputation hazard while using a Sheffield cold knife cutting machine (model number C-530, serial number 5397).     b)    This violation was discovered on May 24, 2017, at the Kaufman, Texas facility; where the employer did not provide a guard on Sheffield heat knife cutting machine (model number HC-530, serial number 2147).
