29 CFR 1915.152(b): The employer did not assess its work activity to determine whether there were hazards present, which necessitate the employee's use of personal protective equipment.       Sunset Hornblower, Bay Ship & Yacht, Alameda:  The written hazard assessment did not cover employees performing fire watch duties and who are exposed to arc flash hazards during MIG torching operations. An employee was observed where clear safety glasses instead of appropriately tinted eye protection (Shade 10).  A written hazard assessment had not been performed to include the elements listed below.
