29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements of subpart I of this part were not worn or attached to the boom or basket when working from an aerial lift.  On or about May 30, 2017, an employee was exposed to a fall hazard and fell approximately 40 feet while performing tree cutting activities because a personal fall arrest or travel restraint system was not worn by the employee while in the aerial lift.
