29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    a)  Work Shop - On or about June 6, 2017, employees operating the table saw to perform angle rip cuts without the automatically adjusting hood being left in place.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    a)  Workshop - On or about June 6, 2017, the tension relief fitting on the flexible cord, energized to 220vAC, routed into the switch box of the table saw was disassembled, which placed the tension from the weight of the cord onto the terminal screws.
