29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards. On or about June 20, 2017, employees in the Vend Stretch Department were exposed to burn hazards and cuts while handling the vending coils. The tyvek sleeves provided by the employer did not protect their arms from heat and cuts/punctures. The employer did not select adequate personal protective equipment to protect employees from these hazards. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified. On or about June 20, 2017, employees in the Vend Stretch Department were exposed to burn hazards while handling the vending coils. The gloves provided by the employer did not protect their arms from heat, as the employer did not select heat resistant gloves to protect employees from this hazard. Abatement certification is required for this item in accordance with requirement of 29 CFR 1903.19(c).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area. On or about June 20, 2017, employees in the Paint Department were exposed to skin irritation, dermatitis, and eye irritation when required to work with chemicals such as, DPI-9 Red ink, paints, and Valrust 1609. The employer did not ensure employees were provided effective information and training on the hazardous chemicals in their work area prior to beginning work. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
