29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s). Tube bending department, Brutis tube bender: On or about June 14, 2017 operators were assigned to hand-feed metal tubes into the unguarded point-of-operation. Operators had their fingers and hands exposed to machine caught-in hazards likely to result in serious injury(amputation where a single-hand cycle activating device was provided. No abatement certification or documentation required for this item.
