29 CFR 1926.451(g)(1):  Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:    a)  South Facing Form Scaffold - Employees were observed pouring concrete from a form scaffold which was not equipped with guardrails or other means of fall protection at the face of the work and were exposed to an approximate 20 foot fall hazard to the ground, on or about June 15, 2017.    b)  East Facing Form Scaffold - Employees were observed pouring concrete from a form scaffold which was not equipped with guardrails or other means of fall protection at the face of the work and were exposed to an approximate 20 foot fall hazard to the ground, on or about June 15, 2017.    No abatement certification or documentation required.

29 CFR 1926.451(g)(4)(ii): The top edge height of toprails on scaffolds manufactured or placed in service after January 1, 2000 were not installed between 38 and 45 inches above the platform surface and installed between 36 and 45 inches above the platform surface on scaffolds manufactured or placed in service before January 1, 2000:    a)  South Facing Form Scaffold - Employees were exposed to an approximate 20 foot fall hazard while working from a form scaffold where the top edge of the end rail measured between 25 - 30 inches high, on or about June 15, 2017.      b)  East Facing Form Scaffold - Employees were exposed to an approximate 20 foot fall hazard while working from a form scaffold where the top edge of the end rail measured between 25 - 30 inches high, on or about June 15, 2017.      No abatement certification or documentation required.
