29 CFR 1910.147(a)(2)(i): This standard applies to the control of energy during servicing and/or maintenance of machines and equipment: (a) On or about June 16, 2017, at a workplace located at 711 FM 2468, Center, TX, an employee was standing in the Pad Department , Zone 3 facing the conveyor in Zone 2 and watching repairs getting done on a sensor. The Robot activated in Zone 3 and hit the employee in the back. Lock out/tagout procedures was not used, which exposed the employee to a caught in between hazard.

29 CFR 1910.212(a)(1): One or more machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying, chips and sparks: (a) On or about June 16, 2017, at a workplace located at 711 FM 2468, Center, TX, an employee was standing in the Pad Department , Zone 3 facing the conveyor in Zone 2 and watching repairs getting done on a sensor. The Staubli Robot F13/5ROLC1/A/01, Type-TX340SH activated in Zone 3 and hit the employee in the back. The employee was exposed to a caught-in-between hazard between the robot and the conveyor.
