29 CFR 1926.28(a): The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions.    On or about 16 July 2017, an employee suffered a severe laceration to his left forearm due to a section of annealed glass mirror that came into contact with a steel door frame and had broken as a result.  Employees engaged in glass and material handling were exposed to cuts and abrasions in that the employer did not require proper cut resistant Personal Protective Equipment to be worn during the handling of annealed glass.
