29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed:    On or about July 5, 2017, the employer did not ensure that authorized employees conducted periodic inspection(s) of energy control procedures, at least annually, to ensure that the procedures and the requirement of this standard were being followed. Employees expected to utilize procedures that were not understandable were exposed to caught-in and struck-by hazards.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:      On or about July 5, 2017, the employer did provide adequate training to ensure that employees tasked with set-up (servicing) of wire bending machines acquired the required knowledge and skills to safely perform such tasks in accordance with documented energy control procedures. Employees were exposed to caught-in/crushing hazards.      Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):     On or about July 5, 2017, and thereafter, employees were exposed to amputation and laceration hazards from the unexpected energizing while performing set-up (servicing) on the AIM bending machines to prepare for normal operation and were not required to control hazardous energy sources in the following sequence prior to starting such work:     1. Turn off and shut down the machines.   2. Physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s).   3. Affix lockout or tagout devices to each energy isolating device.   4. Verify that isolation and de-energization of the machines or equipment have been accomplished.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1904.19(d).
