29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points:(a)(LOCATION)(IDENTIFY SPECIFIC EQUIPMENT AND/OR CONDITIONS)(DESCRIBE HAZARDS WHERE NECESSARY)    On or about July 12, 2017:    a) An employee utilizing a DeWalt saw to cut material was exposed to potential electrical shock and or burns when the cord he was using was run through doorways without being protected from damage.       In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.300(a): Hand tool(s), power tool(s) or similar equipment were not maintained in a safe condition:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR EQUIPMENT, INCLUDING SERIAL NUMBER, MODEL, ETC.)(KIND OF WORK BEING DONE)    On or about July 12, 2017:    a) An employee utilizing a DeWalt DW708 12 inch miter saw was exposed to electrical shock and or burns when the saw being used had it handle duct taped on. The control switch is located within the handle potential exposing the employee to contact with live circuits.    b) An employee utilizing a DeWalt DW708 12 inch miter saw was exposed to amputation hazards when the saw he was using did not have a guard that operator properly. The guard was moved to an up position and a machine screw through it to hold it up out of the way while operating.      Simco Interiors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.300(a), which was contained in OSHA inspection number 545398, citation number 1, item number 1a and was affirmed as a final order on September 14, 2012, with respect to a workplace located at 55 Helen Highway in the city of Cleveland GA.

29 CFR 1926.300(b)(4)(ii): The point of operation of machine(s) whose operation exposed an employee to injury was not guarded:  On or about July 12, 2017:  a) An employee operating a DeWalt DW 708 12 inch miter saw, was exposed to amputation hazards when the point of operation was not guarded, while cutting metal tracking material.   Simco Interiors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.300(b)(1), which was contained in OSHA inspection number 545398, citation number 1, item number 1b and was affirmed as a final order on September 14, 2012 , with respect to a workplace located at 55 Helen Highway in Cleveland, GA.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
