29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    A.)  Robert Russell dba Trimble Landscaping and Remodeling, roof of the structure, on or about July 17, 2017 and times prior thereto:  Employees were performing roofing activities that included, but was not limited to, installing the cap on the roof of the structure.  The employees were exposed to a fall hazard of 10-feet to a lower roof, and 22-feet to the ground, and were not utilizing an acceptable means or method of fall protection.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  Robert Russell dba Trimble Landscaping and Remodeling, roof of the structure, on or about July 17, 2017 and times prior thereto:  Employees were performing roofing activities that included, but was not limited to, installing the cap on the roof of the structure.  An employee was sitting on, and working from, a roof bracket scaffold.  The employee was exposed to fall hazard of 10-feet to a lower roof, and 22-feet to the ground.  The employee was not utilizing an acceptable means or method of fall protection.
