29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     East Side: On or about July 20, 2017, and at times prior thereto, an employee was exposed to a fall hazard greater than 6 feet while using a 16 foot ladder, with side rails extended less than 3 feet from the landing surface, to access the residential roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  East Side: On or about July 20, 2017, and at times prior thereto, an employee without fall protection was exposed to a fall hazard greater than 6 feet to the unpaved ground below while installing sheathing on roof.     Bernabe Montejo Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1197919, citation number 01, item number 001 and was affirmed as a final order on 3/22/2017, with respect to a workplace located at 8235 Pickle Oak, San Antonio, Texas 78223.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:  East Side: On or about July 20, 2017, and at times prior thereto, an employee was exposed to the hazard of being struck by flying projectiles, while using a pneumatic nail gun to install sheathing on the roof without the use of eye protection.
