29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Facility, 633 Napor Boulevard, Suite 200, Pittsburgh, PA: On or about July 24, 2017 the employer did not have an energy control program. Employees perform maintenance on equipment such as, but not limited to: Modern Process Equipment grinders and sifters, Flexicon augers and hoppers and the Donaldson Torit dust collector. The employer does not have energy control procedures and has not conducted any employee training to ensure employees who work on the equipment are not exposed to the unexpected startup or energizing of equipment.

29 CFR 1910.178(m)(4): The employer did not prohibit arms or legs from being placed between the uprights of the mast or outside the running lines of the truck: (a) Facility, 633 Napor Boulevard, Suite 200, Pittsburgh, PA: On or about July 17, 2017 an employee was riding on a pallet on the forks when the operator lowered the mast. The employee had their right hand on the mast and their right pointer and middle fingers were pinched between the mast and frame. The employee sustained an amputation to the first knuckle of the right pointer finger.
