On or about 8/2/2017, at the press brake department of the above addressed jobsite, an employee was exposed to a caught by hazard while working with a press brake (i.e. Omada, Model RG-80, Serial #5050010) that had its point of operation not guarded.      b.  On or about 8/2/2017, at the press brake department of the above addressed jobsite, an employee was exposed to a caught by hazard while working with a press brake (i.e. Omada, Model RG, Serial #804673) that had its point of operation not guarded.      c.  On or about 8/2/2017, at the welding department of the above addressed jobsite, an employee was exposed to a caught by hazard while working with a lathe (i.e. Cadillac, Model 1440C) that had its point of operation not guarded.    d. On or about 8/2/2017, at the model shop of the addressed worksite, an employee was exposed to a caught by hazard while working with a drill press (i.e. Clausing, Model 1634, Serial #124123) that lacked a guard on its point of operation.     e. On or about 8/2/2017, at the model shop of the addressed worksite, an employee was exposed to a caught  by hazard while working with a drill press (i.e. Clausing, Model 1634, Serial #124154) that lacked a guard on its point of operation.     f. On or about 8/2/2017, at the model shop of the addressed worksite, an employee was exposed to a caught by hazard while working with a drill press (i.e. Clausing, Model 1676, Serial #125412) that lacked a guard on its point of operation.     g. On or about 8/2/2017, at the model shop of the addressed worksite, an employee was exposed to a caught by hazard while working with a drill press (i.e. Clausing. Model 1655, Serial #125413) that lacked a guard on its point of operation.     h. On or about 11/8/2017, at the machine shop of the addressed worksite, an employee was exposed to a caught by hazard while working with a drill press (i.e. Orbit Machine Tool, Model OR-1412F, Serial #4531) that lacked a guard on its point of operation.
