29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Centerless Face Lap- Employees are not protected from amputation and severe laceration hazards where the points of operation on the centerless grinding machines were not guarded. On or about July 24, 2017 an employee sustained a partial finger amputation when they contacted the point of operation on KC03 centerless grinding machine during operation.
