29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)    A.)  Double R Roofing, LLC, 12 East Dobbins Landing, Erie, PA  16507, on or about August 8, 2017 and times prior thereto:  Employees were using chemicals including, but not limited to, Conklin Benchmark Acrylic Latex Roof Coating Base Coat.  Employees were not trained on the physical and health hazards of the chemicals.    B.)  Double R Roofing, LLC, 12 East Dobbins Landing, Erie, PA  16507, on or about August 8, 2017 and times prior thereto:  Employees were using chemicals including, but not limited to, Conklin Benchmark Acrylic Latex Roof Coating Base Coat.  Employees were not provided with information on the location of the hazard communication program.

29 CFR 1910.1200(g)(8):  The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):  A.)  Double R Roofing, LLC, 12 East Dobbins Landing, Erie, PA  16507, on or about August 8, 2017 and times prior thereto:  Employees were using chemicals including, but not limited to, Conklin Benchmark Acrylic Latex Roof Coating Base Coat.  The SDS for the chemicals were not on-site.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    B.)  Double R Roofing, LLC, 12 East Dobbins Landing, Erie, PA  16507, on or about August 8, 2017 and times prior thereto:  Employee was performing roofing activities that included, but was not limited to, spraying/brushing Conklin Benchmark Acrylic Latex Roof Coating Base Coat onto the roof surface.  The employee was exposed to a fall hazard of 24.9-feet to the ground below.  The employee was not utilizing a means or method of fall protection.
