29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces:    (a)  At the SNK Milling Machine, on or about August 11, 2017, an employee accessed the top of the machine to a height of approximately 12 feet by climbing the equipment on the side of the machine.  Access and egress was performed in the same manner exposing the employee to a fall hazard.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    (a) At the SNK Milling Machine, on or about August 11, 2017 and at times prior to, an employee was working on the top of the machine to a height of approximately 12 feet without a guardrail system, safety net system, or personal fall arrest system exposing the employee to a fall hazard.

29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    (a) In the De-burr Area, on or about August 11, 2017 and at times prior to, a hearing conservation program was not implemented for an employee exposed to noise at 214% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 90.4 dBA during the 477 minute sampling period, resulting in a noise hazard.    (b)  In the De-burr Area, on or about August 11, 2017 and at times prior to, a hearing conservation program was not implemented for an employee exposed to noise at 212% of the action level (8-hour time-weighted average sound level of 85 dBA) or an equivalent sound level of 90.4 dBA during the 379 minute sampling period, resulting in a noise hazard.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  (a) Throughout the facility, on or about August 10, 2017 and at times prior to, the employer did not provide basic advisory information contained in Appendix D to employees who used filtering face piece respirators on a voluntary basis, resulting in a respiratory and dermal hazard.
