29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Turning Bay: On or about August 11, 2017 and times prior, the employer did not ensure that machine guarding was provided on a U.S. Industrial Machinery lathe (Serial No. K5119398) to protect employees from the rotating chuck.    (b) Turning Bay: On or about August 15, 2017 and times prior, the employer did not ensure that machine guarding was provided on a Yamazaki Machinery Works, LTD lathe (Serial No. 33-045 W) to protect employees from the rotating chuck.    (c) Repair Bay: On or about August 15, 2017 and times prior, the employer did not ensure that machine guarding was provided on the U.S. Industrial Machinery lathe to protect employee from the rotating chuck.    (d) Repair Bay: On or about August 15, 2017 and times prior, the employer did not ensure that machine guarding was provided on a Yamazaki Machinery Works, LTD lathe to protect employees from the rotating chuck.    (e) Repair Bay: On or about August 15, 2017 and times prior, the employer did not ensure that machine guarding was provided on a Yamazaki Machinery Works, LTD lathe (Serial No. 32888 W) to protect employees from the rotating chuck.    (f) Saw Bay: On or about August 11, 2017 and times prior, the employer did not ensure that adequate machine guarding was provided on a WF Wells and Sons, Inc. horizontal bandsaw to protect employees from the unused portion of the blade.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) Turning Bay: On or about August 1, 2017 and times prior, an employee operated the Baldor Pedestal Grinder (ID 1215W) that was equipped with an adjustable tongue guard but was positioned such that the opening between the wheel and the peripheral member was approximately 7/8 of an inch.   (b) Grinding Bay: On or about June 1, 2017 and times prior, an employee operated a Baldor Pedestal Grinder (Serial No. F975) that was not equipped with an adjustable tongue guard allowing for an opening between the wheel and the peripheral member of approximately 7/8 of an inch.
