29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     526 Oak Street, Webster Groves: Employer is exposing employees to hazardous condition.  The employees are permitted to use portable extension ladder with side rails not extended minimum 3 feet above landing surface exposing the employees to fall hazards.    On and about 08/15/2017 a Louisville 24 foot aluminum ladder was used to access a roof approximately 11 feet 11 inches from ground level with side rails extended approximately 1 foot 9 inches above the landing surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):      526 Oak Street, Webster Groves: The employer is exposing employees to hazardous conditions.  The employees are exposed to fall hazards where they work without fall protection systems on roofs higher than 6 feet from the eaves of the roof to ground level.   On or about 08/15/2017 an employee was exposed to a fall distance of approximately 11 feet 11 inches where the employee worked on a roof without using any forms of fall protection systems.  There was no alternative fall protection measure in use.   The Biebel Brothers, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 977327, citation number 1, item number 01 and was affirmed as a final order on 08/13/2014 with respect to a workplace located at 10121 Manchester Road, Saint Louis, MO 63122.
