29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:  At the jobsite located at 6514 83rd St., Overland Park, Kansas 66204: Employees engaged in tearing off the old roofing material activities from a 5x12 pitch roof were exposed to fall hazards of approximately 8 feet 9 inches from the edge/eave of the roof to the ground below in that fall protection was not utilized.  29 CFR 1903.19(c)(2) abatement of the above violation was verified at time of inspection, no certification is required.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the jobsite located at 6514 83rd St., Overland Park, Kansas 66204: Employees engaged in tearing off the old roofing material activities from a 5x12 pitch roof were exposed to fall hazards of approximately 8 feet 9 inches from the edge/eave of the residence to the ground below while accessing and exiting the roof in that the portable ladder did not extend at least 3 feet above the upper landing and was not secured to the structure.  29 CFR 1903.19(c)(2) abatement of the above violation was verified at time of inspection, no certification is required.
