29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a) On 08/23/17, on the flat front canopy roof of the Save-A-Lot  Store, Abbott Road Plaza , 1234 Abbott Road, Lackawanna, NY: Four employees engaged in roofing work were exposed to a fall of approx.  25 ft. to the ground.The employer chose to use a personal  fall arrest system as the fall protection method on this jobsite. However, three of the employees, including the foreman were wearing a full body harness, but none were tethered to an anchor point.The fourth employee was not even wearing a harness.      NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    a) On 08/23/17, on the lower roof of the Save-A-Lot  Store, Abbott Road Plaza , 1234 Abbott Road, Lackawanna, NY: An extension ladder was set up on the south side of the building for access to the lower roof of the building. Once on the lower roof, employees  walked north along a pathway next to an approx. 6 ft. high wall on their right side (east side), until they reached a 6 ft. stepladder propped up against the wall near the end of the path. The employees climbed up this ladder to access the high roof. In doing so, the employees went on the top step and the top of this ladder.     NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.251(c)(5)(i): The U section of U-bolt wire rope clip(s) used to form eyes was not in contact with the dead end of the rope:  a) On 08/28/17, on the upper roof of the Save-A-Lot Store, Abbott Road Plaza , 1234 Abbott Road,  Lackawanna, NY: An extension ladder was set up on the south side of the building for direct access to the high area of the roof. Once on the roof, employees  tied off to a wire rope static line which ran from that ladder access point, northwest to a stepladder where they disconnected from the static line and reconnected to the rope grab on a lifeline that was connected to an anchor point on the canopy. At the north end of the static line, three U-bolt Wire Rope Clips were used to form an eye. The U section on all 3 clips was in contact with the LIVE end of the rope, compressing and damaging the rope.   ABATEMENT CERTIFICATION REQUIRED
