29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    On or about 8/31/2017, and at times before, the employer failed to ensure that employees manually removing rebar mesh from the MG215 (Typ 32.14/16D Nr GS1.4174.5622 & Typ ZG 32 QKVI/1 Nr GS1.4516.5622) utilized lockout procedures. At times when the MG215 timed out due to an error in the programming, or the hooks missed when picking up the rebar mesh to advance it, employees were climbing onto the Typ ZG 32 QKVI/1 Nr GS1.4516.5622, to gain access to the wire rebar mesh to remove the mesh by hand without following lockout procedures as required by 29 CFR 1910.147(d)(1)-(6) as follows:    1) Preparation for shutdown;  2) Machine or equipment shutdown;  3) Machine or equipment isolation;  4) Lockout device application;  5) Relieve/disconnect/render safe stored energy, and;  6) Verification of isolation.
