29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) 153 Ward Drive, Claysburg, PA: On or about July 19, 2017, employees were exposed to the point of operation on the NBX machine; an employee amputated a finger while cleaning off a deposit head.

(a) Facility- 153 Ward Drive, Claysburg, PA: On or about July 19, 2017, the employer did not report an employee amputation within twenty-four hours of knowing the amputation occurred; the amputation occurred on July 19, 2017 and was reported on August 31, 2017.
