29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  On October 27, 2017, Robert Smith, dba Smith Remolding Services, was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:                   a.  Citation 1 Item 1 and,              b.  Citation 1 Item 2.  Robert Smith, dba Smith Remolding Services, was previously cited for a violation of this occupational safety and health standard or its equivalent standard29 CFR 1903.19(d)(1), which was contained in OSHA inspection number 943298, citation number 02, item number 002 and was affirmed as a final order on march 4, 2014, with respect to a workplace located at 701 - 735 Mallard Way, Washington, IL.

29 CFR 1926.102(a)(2): Eye and face protective equipment being worn by the employees did not meet the requirements specified in American National Standards Institute, Z 87.1-1968, Practice for Occupational and Educational Eye and Face Protection:     On or about September 8, 2017, employees were exposed to a struck-by hazard in the eye while using a pneumatic nail gun, on a construction site in Germantown Hills, IL, and the employer did not ensure the sunglasses met the requirements of Z 87.1-1968.            Robert Smith dba Smith Remodeling Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(2), which was contained in OSHA inspection number 895647, citation number 1, item number 1, and was affirmed as a final order on April 29, 2013, with respect to a workplace located at 1285 Independence Court, Washington, IL, 61571.

On or about September 8, 2017, employees were exposed to a fall hazard of approximately 20 feet on a construction site in Germantown Hills, IL, and the employer did not ensure the use of fall protection.      Robert Smith dba Smith Remodeling Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 943298 citation number 1, item number 3, and was affirmed as a final order on December 3, 2013, with respect to a workplace located at 701-735 Mallard Way, Washington, IL 61571.
