29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    The south side of the welding shop: On or about September 27, 2017, and at times prior thereto, employees were exposed to electrical hazards from using a Scotchman Industries Band Saw SH1419 that was missing a grounding pin on the electrical cord for the pump of the coolant system.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use:  In the middle of the welding shop: On or about September 27, 2017 employees were exposed to fire hazards, when an oxygen cylinder and an acetylene cylinder were stored in a storage cart without the valve protection caps affixed.
