29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances shall be allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways shall be kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways shall be appropriately marked: a. On or about the week of September 25, 2017, in the main powered industrial truck (PIT) aisle adjacent to the shipping/staging area, sufficient safe clearances were not maintained for PIT traffic such as Crown turret trucks when pallets of product were permitted to be staged and audited in the PIT aisle which removed the separation of the PIT and pedestrian traffic. On or about September 27, 2017, an employee auditing a pallet of product temporarily staged in the main PIT aisle was struck by a Crown turret truck injuring the employee's right foot.

29 CFR 1910.178(n)(6): The driver shall be required to look in the direction of, and keep a clear view of the path of travel: a. On or about September 27, 2017, an employee who was operating a Crown turret truck (TSP 6000 Series), while backing out of the tower replenishment aisle into the main powered industrial truck (PIT) aisle, was not looking in the direction of, and keeping a clear view of the path of travel, thereby hitting and injuring an employee auditing a pallet of product temporarily staged in the main PIT aisle.
