Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: Jobsite Employees were exposed to falls greater than 10 feet to the ground below, while working on a second story level of a structure, where fall protection had not been used.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds. Location: Jobsite Employees were exposed to fall hazards where anchorage points were not capable of supporting at least 5,000 pounds per employee where: Instance 1) Employees had used choker straps wrapped through wooden parallel chord trusses with light gauge metal gusset plates that were incapable of supporting lateral forces and loads.

The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.
