29 CFR 1910.212(a)(1):  Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    On or about September 10, 2017, employees were exposed to caught-in and amputation hazards from a rotating shaft on a Menzel winder, while performing a line cut-over procedure for 70-mil plastic, on line 8. The employer did not protect employees from unexpected startup of components by the existing system (light curtains) which was defeated and did not provide effective means of protection.      1) Machine or equipment shutdown   2) Machine or equipment isolation   3) Lockout or tagout device application   4) Verification of isolation      The use of light curtains in lieu of Lockout/Tagout did not provide effective alternate protection.    Ex-Tech Plastics, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d) which was contained in OSHA inspection number 1235625, citation number 1 , item number 1, Group B, and was affirmed as a final order on 7/20/2017 with respect to a workplace located at 11413 Burlington Road, Richmond, IL 60071.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19 (d)
