Throughout the production area, on or about 10/11/17:  Affected employees working in areas where equipment is subject to servicing and/or maintenance under lockout-tagout have not received training regarding the energy control procedures in use within the facility. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a.  In the mixing room, on or about 9/15/17:  Employee working in proximity to unguarded rotating parts in a mixer.  Employee came in contact with rotating augers and suffered serious injury.    Abatement certification must be submitted for this item.
