The wheels of the belt sander and the unused portion of the sanding belt were not guarded to prevent hazards of skin abrasion, in-running nip points, and rotating end shafts.  B.)    Nutec Tooling Systems, Inc., 18114 Research Drive, Meadville, PA  16335, band saw area, on or about October 13, 2017 and times prior thereto:  Employees utilize the Kalamazoo Industries one-inch bench top belt sander for operations including, but not limited to, deburring parts.  The wheels of the belt sander and the unused portion of the sanding belt were not guarded to prevent hazards of skin abrasion, and in-running nip points.  C.)    Nutec Tooling Systems, Inc., 18114 Research Drive, Meadville, PA  16335, band saw area, on or about October 13, 2017 and times prior thereto:  Employees utilize the Roll-In Band Saw (Asset #182) for operations including, but not limited to, cutting metal stock.  The band saw was not equipped with an adjustable guard to protect the unused portion of the band saw blade and the blade saw guide wheels.    D.) The band saw door was not complete in that it did not completely protect the band saw blade on the return side.    E.) Nutec Tooling Systems, Inc., shipping and receiving area, on or about October 13, 2017 and times prior thereto:  A Kidde multi-purpose dry chemical fire extinguisher (M/N: PRO 10 TCM-2, S/N:  KL-137498) was located within 6-inches of items stored on the counter in shipping and receiving.  The location of the fire extinguisher did not make the fire extinguisher readily accessible.
