29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    Fabrication area - On or about October 9, 2017, press brake operators using a two-hand control were required to hand-feed the work piece (Gallon Wall Rack Bracket) into the point-of-operation of the Verson Press Brake No. PB50-48. The Verson Press Brake No. PB50-48 did not have an effective safe guarding system such as a anti-repeat feature to prevent unintended  / repeat cycling of the ram during operation. Also the control system for the Verson Press Brake No. PB50-48 did not require the concurrent pressure from both hands on to the two-hand controls during the closing portion of the ram. Employees were thereby exposed to the hazards associated with moving parts.
