29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:      Main working area: A yellow extension cord was directly plugged in a wall outlet that was not provided with a ground fault circuit interrupter for personal protection.

29 CFR 1926.405(g)(1)(iii)(C): 29 CFR 1926.405(g)(1)(iii)(C): Flexible cords and cables are prohibited from being used through doorways, windows, or similar openings except where permitted in paragraph (a)(2)(ii)(1) of this section.    Main working area: An energized yellow extension cord was run through the main working area door to the outside area. The extension cord was pinched by the door frame and the door exposing employees to electrical hazards.
