On or about October 13, 2017, the employer did not ensure procedures were developed, documented, and utilized for the safe application, usage, and removal of potentially hazardous energy controls during the cleaning of the #2 bulk loading screen. Energy sources include, but not limited to, electric power.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: 1910.147(d)(1)-(d)(6). On or about October 13, 2017, an employee was exposed to machine hazards associated with moving and rotating parts while cleaning the #2 bulk loading screen. The employer did not ensure employees implemented energy control procedures by turning off or shutting down equipment [per the 1910.147(d)(3) requirements], isolating all energy sources [(d)(3)], affixing a lockout device to each energy isolating device [(d)(4)], and verify the equipment energy was isolated and de-energized prior to starting work [(d)(6)].

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. On or about October 13, 2017, the employer did not ensure periodic inspections of energy control procedures were being performed at least annually to ensure the requirements of the standard were being followed. The employer did not ensure an inspection was conducted of each energy control procedure to correct any inadequacies or deviations in the procedure [per 1910.147(c)(6)(i)(B) requirements], and/or annual periodic inspections included a review of each authorized employee and their responsibilities under the energy control procedure [(c)(6)(i)(C)].
