29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures.   (a)  At the work site, Column G5; electrical cords were placed along the passageway; on or about 10/20/17.   Note: The employer is required to submit abatement certification for this item accordance with 29 CFR 1903.19.

29 CFR 1910.134(f)(2):  Employees using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator (Construction reference 1926.103):  a)   At the worksite - Employees are required to wear North half mask respirator without being fit tested prior to the initial use, on or about 10/20/17  Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
