29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms and walking-working surfaces were not kept clean, orderly and in sanitary condition. (a) Hotel Rooms - On or about October 24, 2017 the breakfast room, maintenance room, kitchen and guest laundry room were not kept in a sanitary condition in that mold and mildew growing on the ceiling tiles.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed. (a) At the worksite - On or about October 24, 2017, in the maintenance room, an employee was exposed to hazard of electric shock when using the electrical box panel to turn off and on the phone system. The panel had a breaker missing.
