(Near Wheaton, MO.) - On October 25, 2016, the employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to the safe operations and hazards associated with working from elevated work platforms and associated equipment. As a result, one employee received serious injuries and was hospitalized, while the other employee was treated and released.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: Majestic Milling Feed Mill 13163 Farm Rd 1005 - Exeter, MO. (Near Wheaton, MO.) - On October 25, 2016, employees engaged in construction activities were exposed serious injuries, in that a Caterpillar Compact Loader (Skid Steer) was being used to support an elevated makeshift work platform. The platform was a Shipping Container Box that was not specifically designed and approved by the manufacturer for this task. As a result, the platform tipped causing the employees to fall to the concrete surface below. One employee received serious injuries and was hospitalized, while the other employee was treated and released.
