29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: a.) On or about November 1, 2017, the employer did not assure periodic audits of the energy control procedures were completed at least annually. Employees are required to perform servicing and maintenance activities on machines including but not limited to, the G machine, which require energy control procedures that have not been reviewed for deficiencies.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a.) On or about October 25, 2017, employees were exposed to amputation injuries in that the G machine was not locked out prior performing unjamming activities. An employee received a partial finger amputation while attempting to unjam plastic bags near the Delta sealer. The employer did not ensure that energy control application steps to control the electrical and pneumatic energy sources were implemented prior to maintenance tasks the employer did not: � Physically operate energy isolating devices to control energy sources and affix lockout device(s). � Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. � Verify that isolation and deenergiziation of the machine or equipment had been accomplished. Liqui-Box Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d) which was contained in OSHA inspection number 1158595, citation number 1, item number 1b and was affirmed as a final order on September 1, 2016 with respect to a workplace located at 1817 Masters Ave. Ashland, OH 44805.

On or about November 1, 2017, the employer did not ensure employees were adequately trained in the purpose and function of the energy control program. Employees were required to perform activities such as unjamming bags from inside the G machine near the Delta Sealer, and are exposed to amputation hazards from moving parts in that the equipment was not properly deenergized. Liqui-Box Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(7)(I) which was contained in OSHA inspection number 1158595, citation number 1, item number 2 and was affirmed as a final order on September 1, 2016 with respect to a workplace located at 1817 Masters Ave. Ashland, OH 44805.
