29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about November 14, 2017, employees were allowed to unjam and perform servicing and/or repairs to an Tech-Con Automation (Auto-Loader) located within the dock area. The employer did not ensure that specific lockout tagout procedures were developed, documented and utilized in order to eliminate or reduce potential amputation hazards to the hands, fingers, and body. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d)(3): All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). On or about November 14, 2017, employees were allowed to apply locks at the gates while unjamming and providing servicing and/or repairs to an Tech-Con Automation (Auto-Loader) located within the dock area. The employer did not ensure that switches located on each autoloader gate were capable of isolating the machine or equipment in order to eliminate or reduce potential amputation hazards to the hands, fingers, and body. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
