29 CFR 1910.215(a)(4): On off hand grinding machines, a work rest was not available to be used to support the work and the work rest available was not adjusted closely to the wheel with a maximum opening of one-eighth inch.   In the draw press machine area, the employer did not ensure the right work rest was serviceable to be adjusted one-eighth inch from the General Machinery bench grinder wheel prior to operating the bench grinder, exposing employees to struck-by hazards.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch.     In the draw press machine area, the employer did not ensure the left adjustable tongue guard for the General Machinery bench grinder was installed prior to operating the bench grinder, exposing employees to struck-by hazards.

At the draw press machine, the employer did not ensure that the floating stripper plate (ring) was secured from lifting with the die while the machine was in operation exposing employees to amputation hazards.
