29 CFR 1926.50(c): A person who has a valid certificate in first-aid training was not available at the worksite to render first-aid.  On or about 7 November 2017, and at times prior thereto, two employees were delivering new bundles of shingles to a residence under construction in Liberty Hill, Texas without Emergency Medical Services within 4 minutes away and without the crew being 1st Aid/CPR trained and certified.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  On or about 7 November 2017, and at times prior thereto, an employee delivering bundles of shingles to a residence under construction was doing so while near the roof edge without the use of head protection, exposing the employee to the hazard of being struck by falling tools, debris, or materials.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.  On 7 November 2017, an employee suffered a workplace injury that required hospitalization, which was not reported to the local area OSHA office within 24 hours.
