29 CFR 1910.134(i)(1)(ii): The employer did not ensure that compressed air used for respiration accords with the following specifications: Compressed breathing air shall meet at least the requirements for Grade D breathing air as described in ANSI/ Compressed Gas Association Commodity Specification for Air, G-7.1-198.  (a) Employees engaged in blasting or painting activities mandatorily to utilize supplied air hood were exposed to health hazards in that level of oxygen content was not monitored.  (b) Employees engaged in blasting or painting activities mandatorily to utilize supplied air hood were exposed to health hazards in that level of hydrocarbon content was not monitored.  (c) Employees engaged in blasting or painting activities mandatorily to utilize supplied air hood were exposed to health hazards in that level of carbon dioxide was not monitored.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:  Employees engaged in welding activities located at east wall of weld shop were exposed to fire hazards in that oxygen cylinders were stored next to flammable cylinders containing acetylene.  The fire wall which separated the cylinders included a void which resulted in the fire wall not being at least 5 feet high with a fire rating of at least one-half hour.

29 CFR 1910.1026(d)(2)(iii): If monitoring reveals employee exposures to be at or above the action level, the employer shall perform periodic monitoring at least every six months.  On or about December 11, 2017, an employee welding stainless steel in the weld shop and was exposed to hexavalent chromium at an eight-hour time-weighted average level of 12.8 micrograms per cubic meter, approximately 5.12 times over the action level of 2.5 microgram per cubic meter, derived from one sample collected over 480 minutes period.
