29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  (a) On or about November 28, 2017 the written procedures for slitter machine S-8 did not outline the steps for isolating, blocking, and securing all sources of hazardous energy, including pneumatic energy sources, exposing slitter machine operators replacing blades to hazards associated with the unexpected release of energy.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  (a) On or about November 28, 2017 employees were exposed to hazards associated with the unexpected release of energy by not affixing a lockout/tagout device to the electrical power source while changing the blades in slitter machine S-9;  (a) On or about November 28, 2017 employees were exposed to hazards associated with the unexpected release of energy by not affixing a lockout/tagout device to the pneumatic power source while changing the blades in slitter machine S-9.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she began work, and did not remove those devices when he or she stopped working on the machine or equipment being serviced or maintained:   (a) On January 10, 2018 two employees were exposed to hazards associated with the unexpected release of hazardous energy while changing the blades of slitter machine S-8 without each applying a separate lockout device to sources of electrical energy;    (b) On January 10, 2018 two employees were exposed to hazards associated with the unexpected release of hazardous energy while changing the blades of slitter machine S-8 without each applying a separate lockout device to sources of pneumatic energy;    (c) On January 10, 2018 two employees were exposed to hazards associated with the unexpected release of hazardous energy while changing the blades of slitter machine S-9 without each applying a separate lockout device to sources of electrical energy;    (d) On January 10, 2018 two employees were exposed to hazards associated with the unexpected release of hazardous energy while changing the blades of slitter machine S-9 without each applying a separate lockout device to sources of pneumatic energy.
