Worksite located at 9590 N. McGee St., Kansas City, Missouri- Employees were exposed to a fire or explosion hazard in that the flammable container being used by the employees was not an approved metal safety or DOT approved container.

Worksite located at 9590 N. McGee St., Kansas City, Missouri- Employee was observed working on the edge of the roof structure and was not protected from falling by the use of any means of fall protection.

Worksite located at 9590 N. McGee St., Kansas City, Missouri- Employee was exposed to a fall hazard in that the ladder being used to access the upper landing surface was not extended at least 3 feet above the upper landing surface.
