On or about December 07, 2017, the corner of 107th Avenue North and 7th Street North, Naples Florida 34101, employees were permitted to work in an 11-foot deep trench excavation with type C soil, that did not have adequate cave-in protection in that employees had to grade soil inside a trench box which used a single road plate which did not cover the entire width and height of the trench excavation and was not designed for cave-in protection by a professional engineer, thus exposing employees to a caught-in between hazard.      b. On or about December 07, 2017, the corner of 107th Avenue North and 7th Street North, Naples Florida 34101, employees were exposed to a to a caught-in between hazard where a 6-foot deep section of the trench excavation did not have cave in protection as they descended a ladder to gain access to a manhole.
