29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   On or about at 12/13/17 located at 4645 Roan Ruin, Cumming, GA: Employees were using scaffold components to access the varying levels of the scaffold instead of being provided with a ladder or other means of access.

29 CFR 1926.451(b)(1)(ii): The platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm):   On or about at 12/13/17 located at 4645 Roan Ruin, Cumming GA:  The employer failed to ensure that the scaffold were fully decked, planks were missing his employees were unprotected from a fall of 22- foot to the lower level while installing bricks onto the walls of the house that was under construction.   The employer has been made aware that the conditions violated a standard because the employer has been previously cited for violations of the same or similar fall protection standards.  Chilo Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926. 451(b)(1)(ii):, which was contained in OSHA inspection number 993486, citation number 1, item number 1, and was opened on 9/2/2014, and the FOD date 10/6/14 with respect to a workplace located at 5340 Pine Hill Way, Cumming GA 30040.
