29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  On or about December 12, 2017, an employee was exposed to fall hazards from a frame scaffold and there was approximately 5-inches of space between the 12-inch plank and the outermost uprights.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered only under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling, or alteration and such activities were not performed by only experienced and trained employees:  On or about December 12, 2017, employees used a frame scaffold that was not inspected by a competent person and employees were exposed to fall hazards when they used the scaffold to:  a) access the roof using the ladder on the scaffold; and  b) transferred sheet metal between the gravel driveway and roof.

On or about December 12, 2017, employees were exposed to injuries and the employer provided the employees a frame scaffold to access the roof with a break in elevation greater than 19-inches between the roof and scaffold plank.
