29 CFR 1910.22(d)(2): (2) Hazardous conditions on walking-working surfaces were not corrected or repaired before the employee uses the walking-working surface.  If the correction or repair cannot be made immediately, the hazard must be guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired;   (a) In the maintenance shop near the machinery employees were exposed to fall hazard from the floor opening measuring 18inches X 6 inches and 4 inches deep, on or about 12/12/2017.
