Employees worked on a MetalTech mobile scaffold platform with unprotected sides or edges that was approximately 10 feet high.  This scaffold had no guardrails lengthwise on both sides.  Personal fall arrest systems, when used, were not designed, installed, and used as a part of a complete personal fall arrest system which maintains a safety factor of two, and were not rigged such that employees could neither free fall more than 6 feet, nor contact the lower level.  This condition exposed employees to fall hazards of approximately 10 feet.    b)  Signature Rattan LLC 8356 Syracuse Street, Commerce City, CO 80022:  On and before November 29, 2017 the employer did not ensure that each employee working on pallets raised by the Hyster H60C forklift was protected from falling in accordance with 29 CFR Part 1926, Subpart L.  The pallet platforms used to elevate employees were not designed by a qualified person to be used as mobile scaffolds with this forklift.  This condition exposed employees to fall hazards of up to approximately 20 feet.
